If you are visiting Dr. Mimi Lee for the first time or for a new problem, please arrive 15 minutes before your scheduled appointment to allow time for registration. We request you to complete our patient forms online at least 48 hours prior to your appointment due to time required to process these forms.
To help Dr. Lee provide you with the best possible, most efficient care, please bring the following to your first appointment:
- Insurance card(s)
- If your insurance company requires a written referral, bring the referral to your office visit
- A list of current medication(s)
- Co-payment, if applicable
- If you choose to not complete our patient forms online and requested them by mail, please mail them back to our office at least 48 hours in advance of your appointment.
Please let Dr. Lee’s office staff know about any changes in name, address, telephone numbers and insurance carriers, so we can keep your records current. We require all information be updated annually.
If you are unable to keep a scheduled appointment, please let us know as soon as possible so another patient can be seen during your previously appointed time.
